Transition Planning

School-Connect supports post-secondary goals as students develop self-advocacy skills and independent living skills throughout our curriculum, along with modules dedicated to employability skills, education options after high school, and preparing for adulthood.

Family Discussion Questions & Activities

Family and caregiver involvement is beneficial and made possible through our parent and family resources that include module summaries, discussion questions, and activities to make School-Connect accessible in students’ homes. These resources give parents a better understanding of what their teens are learning and create opportunities for families to connect and find common ground around School-Connect strategies that may benefit them.

S-C Progress Monitoring Assessments

Measure and track skill development with ease and accuracy. Our assessments are comprised of student self-reports and teacher observation forms. The assessment items for each module align with lesson learning objectives and assess observable skills. We suggest using these assessments as pre/ post measures to capture data and track growth.
